Terms and conditions for Body Back + 360 Membership
This plan is non-refundable and non-transferable. This plan may be put on hold for a maximum of 4 months per year, with 1 month being the minimum hold allowed. In order to put your account on hold or to cancel your account, you must fill out our Membership Change Request Form with at least two weeks' notice. This form can be accessed on our website.
You may change your membership to another monthly membership by filling out the same Membership Change Request Form at any time. Please note that downgrading to a lower-priced option mid-month will not result in credit to your account, so we recommend that any downgrades are requested to take effect on the first of the following month.
